All animals must be current on all vaccinations.
All animals must be free of external parasites (ticks, fleas, etc) or they will be treated at owners expense.
Great Oaks Veterinary Hospital has permission to do whatever is necessary should an emergency arise.
If a tranquilizer is necessary for treatment or handling, Great Oaks Veterinary Hospital has my permission to administer such medication.

In case of illness or injury and the emergency contact can not be reached, I undersigned, do hereby give my consent for the doctor of Great Oaks Veterinary Hospital to treat, prescribe for, and/or operate upon my pet while he/she is boarding at Great Oaks Veterinary Hospital
